Support Coordination-Assistance with Life Stage Transitions

including short and long-term supports that focus on strengthening the participant’s ability to coordinate their supports, and to assist them to live at home and participate in their community.
Services include support connection; coordination of supports; assistance with accommodation and tenancy obligations; life transition planning including mentoring, peer support and individual skill development.

Specialist Support Coordination

The provision of Support Coordination within a specialist framework necessitated by specific high-level risks in the participant’s situation. Support is time limited and focuses on addressing barriers and reducing complexity in the support environment, while assisting the participant to connect with supports and build capacity and resilience. It may also involve development of an intervention plan which will be put in place by disability support workers.

Participation within the Community

assisting participants to participate actively in community, social and civic activities. This includes supporting participants during these activities and developing their ability to take part in these activities.

Innovative Community Participation

assisting participant to access mainstream leisure, social or other activities within their community.

Assistance with Personal Activities

assisting and supervising participants with personal tasks of daily life to enable the client to live as autonomously as possible.

Assistance with Daily Tasks in Shared Living

assisting participants with, or supervising tasks of daily life in a shared living environment, which is either temporary or ongoing, with a focus on developing the skills of each participant to live as autonomously as possible. This includes short-term accommodation and assistance (e.g. respite care).

Development of Daily Living and Life Skills

this incudes training and development activities to increase participant’s and carer’s ability to live as autonomously as possible, including enhancing the ability of participants to travel and use public transport independently.

Household Tasks

essential tasks that participants are not able to undertake, such as house cleaning, changing linen, washing, house or yard maintenance.

Every one of our STA trips are 100% customised to assist participants to reach not only their plan goals but their life goals.
